Innovis currently offers two convenient ways to request a copy of your Innovis Credit Report: by phone or by mail.

Phone: Call 1-800-540-2505 anytime to request a copy of your Innovis Credit Report using our automated system. Representatives are available to assist you Monday through Friday, 8:00 a.m. - 8:00 p.m. ET. You will be asked to verify several pieces of information prior to fulfillment of your request. Once your identity has been verified, you should receive a copy of your Innovis Credit Report in three to five business days.

If we are unable to complete identity verification by phone, you may be asked to submit your request by mail.

Mail: Please complete the attached Credit Report Request Form and mail it to Innovis at the address provided below.

Please include the following information when submitting your request in writing:

• Full name
• Social Security Number
• Date of birth (month/day/year)

• Received a notice of an adverse decision - such as a denial of credit, insurance or employment - within the past 60 days.
